You need to spend some time boosting your ego. Do nice things to make yourself look 'nice'. Hair cut, shirts, trousers, shoes. (I assume you're a guy). Keep your money with you rather than make your gp richer. He will simply give you advice any Tom, Dick, or Harry can give.

You need to begin to do other things, like take up voice classes, some acting school, smile to yourself (at appropriate places as well), take up a hobby to trim off excess body fats, etc....

You need yourself to say affirmative things about yourself. There are several self help books that you can buy that teach you this.

Stop hanging around seedy places, or watch, listen to meaningless tv prog, music, etc. A church may even be a great place to strike up some safe conversations without being religious.

You need one best friend. We all do. Foremost, you need to be your own best friend first. Be Your Own Best Friend by Thomas A Whiteman, and Randy Petersen is a good book to begin this process....

Good luck. :)
References :
Tell your doctor. Usually they'll refer you to a therapist, or find one yourself. The therapist usually tries to help you find what it is that sets you off. Be it a fear of something happening, the interaction itself, or if its something chemical (in which case he would recommend a psychiatrist). Its my experience, therapists and psychologists are the better way to go if it isn't a chemical problem. They're usually cheaper, and they're more adept to listening and are generally more empathic (in my experience anyway).
References :
I would recommend you seek medical help either from your PCP or by referral to a psychiatrist. First, they need to exclude medical causes of anxiety. Then they can help you come up with a plan for treatment. Treatment can involve medications (many of the antidepressants are indicated for social anxiety disorder) and also different kinds of therapy. There are lots of behavioral strategies as well as cognitive strategies that you can do to get relief from anxiety disorders that are remarkably effective. Good luck!
References :
personal experience

you can go to a gp first and ask for a referral to a psychiatrist. you can also go to a psychologist first and he'll assess your case and will refer you to a psychiatrist if you need medications. Or you can go to a psychiatrist directly. I guess the advantage of going to a GP is that they can assess if you have an organic problem than a psychological problem. Like for example, if you have palpitations (you can feel your heart beating fast) perhaps the doctor can check your blood for thyroid hormones since hyperthyroidism can present with palpitations. Or if you experience skip beats, perhaps the doctor can order for an ECG of 2D echo to check the status of your heart, like if your have some type of arhyhthmia or valvular problem. But personally i think the psychiatrist should know what to do if he's suspecting an organic problem because I think psychiatrist should exclude organic problems first before making a psychiatric diagnosis and besides he can refer you to another specialist if he cannot handle your case or if he thinks its outside his turf. If you really do have social anxiety, I wish you all the best in trying to overcome it...
References :
i have a fear of crowds. what i do is i go out on quiet days with someone i am close to like my mum or best friend. and i slowly introduce myself to large crowds.

another thing i do, suggested by GP, is to sit in a room with no noise to distract you and think about scenarios where you are afraid of the social life and try to pin point what factors are around you that cause the anxiety and talk it over with someone about why this causes you the most harm.

This is not an easy case to solve this way, i have been trying for years, but i can't afford the psychologists bill so i am doing it the long way. Needless to say the psychologist too can take years to treat.

Hope you get better soon
